Seeing Your Physician

• Masks are still mandatory in the clinic

• please DO NOT come to your appointment if you have any symptoms that could represent COVID-19 including fever, runny nose, cough, chills, fatigue, loss of taste or smell. Please CALL TO CANCEL your appointment

• if you arrive early, please wait in your car and come in at the appointment time

• your physician may want to talk to you on the phone before booking an in person visit. This is to determine if an in person visit is needed, and to make the in person exam if necessary shorter to reduce the face to face time and reduce the risk of the visit to both parties

• please have your email address ready and the name of the pharmacy you would like any prescriptions sent to with you prior to your phone appointment

• if you have a photo of a problem you would like to share with your doctor, please email it to 4oaksmedical@gmail.com within 24 hours of your appointment so that it can be loaded into your file. If you can, place a ruler in the field of the photo so that your doctor can tell how large the area of concern is. Please also ensure the photo is well lit and in focus
